Geriatrics And Prosthodontics The Symbiotic Connection

Introduction:

Geriatrics and the prosthodontic care of our aging patients do play a signifi cant role in managing the oral health of older adults. Older adults have been poor users of dental services. Reasons cited for the lack of utilization include fi nancial constraints and lack of access.  


Expectations And Goals :

Overview of geriatric dentistry, biological aspects of aging and dental management of medically complex patients with dental manifestations with conditions such as Diabetes, Dry mouth, Denture Stomatitis, Candidiasis, GERD, HTN, Osteoporosis, Stroke, Obesity, Alzheimer’s. 

Oral health assessment Tool in older patients, prosthodontic treatments and treatment planning principles in geriatric patients and clinical management. The new CDCP plan and their benefi ts for our aging patients.


Course Objectives:

  1. Importance and scope of practicing geriatric dentistry

  2. Demographic trends and aging population

  3. Medically complex patients and prosthodontic treatment management.

  4. Prosthodontic treatment options and treatment planning and sequence for partially and complete edentulous patients.

  5. Understanding dental conditions that can influence and impact on denture wearing and maintenance. 

  6. Elderly patient education and knowledge of denture hygiene

Fundamental Principles Of Fixed Tooth Supported Restorations

Introduction:

Fixed tooth-supported restorations, such as post and cores, crowns and bridges, are designed to replace or restore damaged or missing teeth with predictable prosthesis. The fundamental principles guiding these restorations ensure their effectiveness, longevity, and comfort. The following principles will be discussed in detail.

  1. Biological Principles

    Preservation of Tooth Structure: maintain tooth structure for strength and vitality. Periodontal Health: Precision of fi t and long-term maintenance. Optimal Periodontal Health Occlusal Considerations: Restorations in harmony with natural occlusion.

    Margin Adaptation and Material Selection: Seamless marginal adaptation

  2. Mechanical Principles

    Retention and Resistance form (RR form): Restoration designs to meet the RR form.

  3. Esthetic Principles

    Aesthetic Integration: Seamless integration with surrounding natural teeth in terms of color, shape, and contour. 

    Proportions and Alignment: Attention to detail for restorations harmony and alignment with white and pink.

  4. Functional and technical (Lab) Principles

    Occlusal Harmony & Efficiency: Effective functional guidance for predictable mutually protected occlusion.

    Accuracy in Preparation and Ceramic selection: Attention to detail in tooth preparation, impression for post and cores, crowns and bridges, clinical protocols for posts and crowns. Proper Fabrication: New trends in digital planning and effective lab communication.

  5. Patient-Centered / Treatment Planning Principles

    Informed Consent/Communication: Effective patient communication. Post Treatment care.

Dr. M.K. Bobby Baig is the Clinical Director of Prosthodontics in the Dept of AEGD, and Associate Professor in the Department of General Dentistry, at University of Rochester, Eastman Institute for Oral Health, Rochester New York, USA. He also works as a Clinical Instructor in Prosthodontics in Mt Sinai Hospital, and Associate In Dentistry at University of Toronto Faculty of Dentistry, Toronto, Canada, and private practice at Dental Specialists Group, Woodbridge and Mississauga, Toronto in Ontario. He is a board-certified Prosthodontist in Canada and serves as a national examiner for NDSE National Dental Specialty Examination board of Canada; and national examiner for Prosthodontics Fellowship F.R.C.D.(C) administered by Royal College of Dentist of Canada. Dr. Baig was awarded the Part time faculty Award for Excellence in Teaching by the University of Rochester, NY, USA for the year 2019, Dr. Baig received Fellowship Award at the International Academy for Dental Facial Esthetics (FIADFE), Nov 2021.

Manhattan New York City. Dr. Baig also received an Award for Excellence in Clinical Education from the Department of AEGD, University of Rochester, Eastman Institute for Oral Health in June 2023.


Dr. Mario Rotella is a Fellow of the Royal College of Dentists of Canada (F.R.C.D.(C)), certified specialist in Prosthodontics. Dr. Rotella focuses his practice concentrating on the treatment of complex cases of aesthetic and functional rehabilitation on natural dentition and implants, with emphasis on veneers, crowns, bridges and dentures.
